Pissman began as dark humour after its creator underwent radical cystectomy and started living with a permanent urostomy.
Jokes about bags, mucus, leaks and an endlessly flowing stoma gradually became a game universe. Its hero is ridiculous, capable and genuinely willing to help.

The comedy targets Pissman, his fictional circumstances, the villains and the machinery of Pisscity. People living with ostomies are not the punchline.
The corrupt doctors are a fictional faction. Aracelle is a doctor and an ally. The world is outrageous; its people still matter.
